Create new record into table via popup in UI Builder

Hi Team,

 when i am working on a UI builder page Consider i am on epic table list if i click on new button a popup should display just like below snip(Only fields from the epic tables not all) and i should have an option to create those records into the epic tables.

Could you help me how to configure this.
