How to additonal fields in Assessment form for Third Part Assessmnet

Hi All,
I have a requirement where I need to add few addtional fields to capture some information in the below screenshot.  I want to add few more text fields similar to reviewers comments. Please help.

Seem like you have shared the screenshot form a Third-Party Assessor profile. 

You should identifying the template applied in the assessment, open that in the Assessment Workspace and try to add questions ( Text box) as per your requirement.

I want additional fields that should be only visible to reviewer .  These fields cannot be  be part of  questionnaire
