Policy Exception Automatically Closed After Extension Approval

I had a policy exception record with an initial Valid to date of 6/1/2026. However, the business owner requested we extend the exception for an additional 11 months, extending the valid to date to 5/1/2027. I approved the extension prior to the original valid to date, but the system automatically closed the record on the initial valid to date. Am I missing something in the workflow? I would have expected it to stay in approved state until 5/1/2027.

requested and approved extension ignored by the system, policy exception has been closed after original valid_to date was reached

do you have more info or tips where to start with finding root cause?
