Policy Exception Issues

We are using GRC - Policy Exception in ServiceNow. Initially, we had custom workflows in place for approvals (including extensions, approvals, etc.), and the OOB (Out-of-the-Box) flows were not used.

Now, we are trying to transition to OOB approval rules for all policy exception processes. Here's what we did:

Disabled all custom legacy workflows.

Configured the Approval Engine with an approval rule for the Policy Exception [grc_policy_exception] table.

Set the engine state to use Approval Rules instead of legacy workflows.

However, we're encountering the following issues:

The system still shows: "Workflows are managing approvals on this table. This should be set to 'Turn Engines Off'"

Approval rules are not being triggered when a new Policy Exception is created.

We’ve verified that the approval rule is active, and there are no custom workflows running on the record.

What could be preventing the OOB approval rule from triggering for Policy Exceptions?
