Unable to Load Questionnaire Templates List for Smart Assessment in External Assessments

Hi everyone,
We are experiencing an issue with External Assessments in our instance.

Issue Description

When creating or editing an External Assessment that uses the Smart Assessment type, we navigate to the Questionnaire Templates related list and click the Add button. However, instead of displaying the available questionnaire templates, the modal window shows the following error:

Unable to load list

As a result, we are unable to add any questionnaire templates to the Smart Assessment.

Steps to Reproduce

Navigate to External Assessments.
Open or create a Smart Assessment type External Assessment.
Go to the Questionnaire Templates related list.
Click Add.
The modal opens but displays "Unable to load list" instead of loading the available records.

Could you please add screenshot of the related list.

I could see only New or Edit button in the Questionnaire templates related list. I checked the UI action for the table, there is no Add UI action for the table.
To update the assessment template, you can go the assessment workspace, select the assessment template and update the template directly which is the usual approach, why do you update from an external assessment ?

Its not recommended to update the Questionnaire template, after an assessment is triggered. If something needs to be changed then we can cancel the external assessment, update and regenerate a new assessment.

Hi @Vinod54, thanks a lot for looking into this and for the suggestion.

I dug into it further and I don't think this is about where we add the templates — it looks like a genuine platform issue with the "Add" (multi-record associator) dialog itself when the related list is empty.

What I tested

I reproduced the exact same "Unable to load the list" error at the Assessment Template level too (Vendor Management Workspace → Assessment template → SAE templates → SAE Questionnaire Templates related list → Add), not just on the External Assessment. So going through the assessment template / workspace hits the same wall.
