How to configure the notifications to show up in the bell icon in the instance.

Even though all the notification preferences are enabled, none of the notification that are triggered in the flow when send notification action is used, where does these notifications end up, how to make them visible in the bell icon widget.

those notification/bell icons mean Provider Notifications

Did you configure that as per customer requirement?

Create provider notification in ServiceNow when mentioned on Ticket 

See below OOTB available on incident table
