Can someone go over document templates?

I created one attached to my custom app scope however I do not see it.   I am expecting to see a ui action to generate the html from the fields on the record, but I do not see anything.    Someone please explain to me how this works.  Much Appreciation.  Thanks, Chad

I was able to generate the template however how to use from your custom table?   Here is what I want to do there are 5 doc templates I want to pick one click the ui action then generate.  What am I missing.
