Please Configure HR Task template as configured in following screenshot. It is important to set State = ready. (You can see this view by clicking Switch to New UI button)

Hi Irfan,
If you are using Madrid build, upload document task type is available OOTB.
It could be hide. To enable, Navigate to HR Case Management -> HR Tasks -> All
Right click on Hamburger icon then configure -> Table
Click on Columns tab, search for Task Type and then select HR Task Type record.
Click on Choices tab, validate Upload Documents record has Inactive = false
Please mark as correct if it resolve your issue or let me know if further explanation needed.
Thanks

I always create lifecycle events in HR core application scope or HR LE Application scope. It is always good to have all related task and activities in same application scope.
Can you show me Screenshots of your Lifecycle event and the HR service Configuration that triggered this lifecycle event and the case which is created?
