In the HR Survey, only one trigger condition is working out of multiple.

Our HR survey is configured with multiple trigger conditions for different COEs. However, only one trigger condition functions at a time. Currently, the "last updated" trigger condition is the only one working and triggering the survey. When any other trigger condition is updated, it starts working, but the previously functioning condition stops working.

 

Has anyone encountered similar issues and can offer assistance?

Please find the screen shot of the trigger condition. We have additional trigger conditions for this survey for various tables that share the same condition.
