Survey Definition form is looking weird

Hello Community,

 

What can be the reason that Survey Definition form is looking so weird like no Name, no Intro /Ending notes , no other fields are visible even on the form even in the list view.

Attaching here is the image for reference.

are you with admin?

Try clearing instance cache using cache.do and then logout and login?

seems some script is hiding those fields.

Already did that but its not working .

if fields are there in form layout then any UI policy or client script is hiding?

are you with admin?

In ServiceNow, different views can be configured to display a specific set of fields and sections, tailoring the interface for different users or tasks. The header of your image indicates the current view is "View: Survey," which is likely a specialized view that excludes fields like "Name" and "Introduction & End Notes" and prioritizes the reporting charts.
