Why multiple notifications are getting triggered at a time?

Hi
i'm facing a issue with twice trigger notifications.
1. When i added a new comments on HR Case notification trigger a twice from same template.
2. When the approval & reject request notifications also same issue triggering twice.
we have updated comments at once only.
Let me know the root cause of it, Thanks in advance!,.

from when did it start happening?
what debugging did you do?

It will be happening in recent times only. I have check the configuration everything looks good only not sure why it triggering a twice.
send to event creator checkbox also enabled. even it will behaves same.
that notification conditions additional comments changes & service is this like wise i have given. notification runs only record gets update (checkbox checkin).

sorry but we don't have access to your instance and can't debug further.
You will have to identify the root cause
check how the notifications are configured to trigger? if the record is getting satisfied with that notification condition multiple times then it will send multiple emails
