help text for incident templates

We're looking to setup some templates for our incidents to help first line triage before escalating calls to different teams/departments and was wondering how best to add help text on a field for example:
Different templates would have different questions, I've seen you can add help text to labels, but I don't see this as a practical solution as some templates could have up to 10 questions depending on the template?
What would be the best way to achieve this.

If you want to display specific template guidelines without prefilling fields like "description" in your example, then you can have a client script that read the short description of the template and shows it as a message.
