The incident is not getting attached as a parent on the Request table.
Whenever we create request using the create request UI action from incident form, ideally it will redirect to self- service service catalog page, from where we will select and order/submit the catalog item.
For some catalog items, it is working; for some, it is not.
Did anyone faced the same issue? Please let me know.
Thank you.

The parent field will be populated only for those Requests created via the 'Create Request' UI action. If a Request is created through the Service Portal, the parent field will always be blank, and that is the expected behavior
If my response proves useful, please indicate its helpfulness by selecting " Accept as Solution" and " Helpful." This action benefits both the community and me.

So did you identify some patterns for which catalog items if request is submitted it doesn't populate parent?
Also are you checking this in native view only and not in workspace?
If my response helped please mark it correct and close the thread so that it benefits future readers.

We are currently working on improving this feature for our customers. The behavior you're seeing is expected when the 'Create Request' button is clicked.
To view the parent field, you need to add it to the Request table—either in the list view or on the form.
