Extracted Fields are not Populating in Table Record

Hi,
I am new to using Now Assist in Document Intelligence and am trying to set it up / test within my team's Demo instance. I am having issues with getting the table record to be populated with the extracted values.
1. I have Now Assist products, Document Intelligence, and Now Assist in Document Intelligence installed with all necessary prerequisites and licensing
2. Skill 'Extract information from documents' is Active
3. I have created a use case, selected a target table, established fields and questions, turned on full automation mode, access has been set to 'Any authenticated user', and it has been activated
4. I have created two integrations, one for document task creation and one for information extraction, both of which are Active
5. I believe document task is being created, I can see a record in sys_di_task_definition and Document Task. When I look at the extract value JSON I see it is populated (apologies for not including this output, I think my clipboard cache has been corrupted and I can't copy / paste currently) with the extracted values.
I have tried to provide as many relevant screenshots as possible. Any assistance would be greatly appreciated, thank you!

I created the Extract Values Flow, but I noticed the values are not written in my target record after the extraction; what’s going on?
First, make sure the values are populated in the Field Values / Extracted Value table [(sys_)di_extracted_value]. Then, ensure the Extract Value Flow is running correctly and the Flow properties are set to run as System User; make sure you see the data in the Flow Execution logs. Finally, check the Table configuration:
- Review ACLs on the Table
- Application access on the Table
- Check the cross-scope privileges
- Make sure the target field type matches the type of data
