Installed MID Server does not connect with our client's instance.

Hi folks,
We have successfully installed a MID Server on our client's Windows server, but it somehow has not appeared on the MID Server table in their instance. After the installation, there appeared a record on the Server Issues table that stated the following statement ("midserver" is the user we created as a preparation for the installation).
"User midserver with mid_server role not associated with a MID Server. No login attempts within reporting period."
When we gave it the second try and installed a new MID Server, it ended up not even connecting to our client's instance, hence no records on Server Issues table. Though we endeavoured as much as possible to research into potential reasons, we are now kind of stuck and looking for help.
All we acknowledge by now are follows:
- We completed the installation process without an error.
- Our client uses a proxy server.
What would you say would be potential causes of this issue, and how could we connect an installed MID Server with our client's instance?

Description
A MID Server issues record for "User X with mid_server role not associated with a MID Server. No login attempts within reporting period." is likely to be created between creating the User and before installing the MID Server.
Steps to Reproduce
In Quebec Patch 1
- Create a new user in the instance, and add the mid_server role
- Intend to install a MID Server within the next hour or so, but don't just yet
- After a short while (which will depend on when the MIDUserConnectivity sysauto-script runs), a MID Server Issues record will be created for the following:
"User X with mid_server role not associated with a MID Server. No login attempts within reporting period."- Install a MID Server using that user
- The issues record remains until the MIDUserConnectivity sysauto-script runs again.
Workaround
There is no issue.
This is the normal order in which the MID Server user is to be created beforehand and then the install is done, as per the docs.
The issue record does get automatically cleared once the MID Servers are installed, but the MIDUserConnectivity job that creates and clears this runs only every 4 hours, and so there time when this error can cause confusion
