Need Gap analysis report format

Hi All

Am doing the Gap analysis for the CMDB on Dev and Prod environment by comparing the both the environments.  I searched for a sample assessment report in now create but could not get it. If you have sample Gap analysis assessment report for CMDB could you please share?
