Service Mapping - uncategorized errors for every service map

We noticed a while back that on all of our Service Maps, every server has a yellow yield sign next to it.
Most of the errors state "Failed to identify the CI Type of the process listening on port xxxx on host xxx.xxx.xxx.xxx. Please review the discovery log for further details." I've also seen the error "Failed to recognize application. See the Discovery Pattern Log for details".
When I click on the Error Handling link for either error, I get the message:
"Only categorized errors can be handled.
To view and rediscover uncategorized errors, navigate to
Service Mapping > Home > Fix."
When I navigate there I see this:
Nothing I do in any of the 4 options helps resolve the issue. This is happening for every service map and I'm not sure what caused it, or how to correct it. Does anyone know how I can correct the issue and successfully discover/map my services again?

Hi @Mike LHCG1,
Here are my thoughts. Few things to check:
Are you able to fully discover these servers using midservers used by service mapping?
Make sure your mid sever ip ranges are accurate
credentials need to have access to the mid severs.
Are there any recent discovery pattern changes/customizations in your instance
Please all validate these configurations.
