A UI Action named 'Create Knowledge' is not displaying on incident. It is however displaying on Workspace but it doesn't work.

I have checked that there is not an action name similar to this one and this exists in the sys_ux_form_action table.

You'd want to review your conditions to see why it's not showing. Consider removing all of them, then checking, then adding each piece back one by one to troubleshoot.

It works fine on Incident form after removing all conditions and also I can see the visibility of the button on Workspace, however it still doesn't work on Agent Workspace.

As far as it not working on agent workspace, please see the agent workspace script section at the very bottom of the UI Action page and work to implement your script there, as it may need alternative code.
