I created a custom table to store key contacts and it has been working for years. I just came back to add one through a related list and clicked new and I get 'action not authorised'. I tried with admin and still get the same message appear. What could this be due to?

Can you check if any recent changes on ACL either on the table you created or parent table if this is inherited custom table.

Issue
UI Actions with script in 'Omit new button' or 'Omit edit button' in the list control throws an error "Action not authorized"
Cause
If "current.parent" is used in the 'Omit new button' or 'Omit edit button' in the list control, as it is known to throw these errors.
Resolution
Use "parent" directly. For example, you can use "parent.sys_id" or "parent.state"
