Catalog Item fulfillment flow is not being triggered

I built a catalog item, but after submission the fulfillment flow is not being kicked off. The request and RITM exist but no approval and no tasks.

Hello @KarsonD ,
can you please check only this flow is not triggering or any other catalog item related flow is also not triggering . Because last time , my colleague all catalog related flows were not trigerring due to Service catalog request ootb workflow was checkout by mistakenly .

Also, if I change the approval field of the request to approved it then kicks off the fulfillment flow. but I didn't think this was how it was supposed to work. The approval happens in the flow that is associated with the catalog item. why would I need to approve the overall request before the flow will be executed.
