Change Approval UI Action Button

I am new to UI action. Could you please help me to achieve this. Thank you

Create UI Action
Name: Change Approval
Table: incident
UI Action should do the Open Dialog-- Dropdown to select approval state and Comment fieldOn Dialog. Submit change approval state to selected add comment provided in dialog to work notes.
