Change approvals are not getting attached

Hi ,

 

We have custom workflow created for change module. Post upgrade from Washington to Yokohama, our approvals are not working. Records are not getting created into approvals. Please suggest.

Is this issue in dev or all instance? Did you check is any skip record which creating issue here?

Has anyone solved this yet?  The workflows are just simply not starting at all even though the conditions are met.  This is happening in all instances.
