We are implementing SNow now and moving to ITIL 4 Our change volume is very high. I'm looking to understand how other organisations have implemented change post implementation review without it being a very large overhead on teh change team and on our customers

Hi Allan,
Completed changes may be reviewed in CAB meetings. The examination of a completed change is called a post-implementation review (PIR). The purpose of a post-implementation review is to study recently completed changes and apply the experience gained to subsequent changes. Post-implementation reviews allow change management to learn from the past and continually improve the change management process.
The Post Implementation tab will be a custom made one.
The OOB tabs are as follows
- Planning
- Schedule
- Conflicts
- Notes
- Closure Information

Did you create each question as field on change request table?
if not, what is the process you followed to create post implementation questions?

As per OOTB, the PIR task get created only for emergency changes, it did not contain any question, but what I saw in my experience, the description contains the set of question which need to answer as part of PIR. Bcz creating new fields will be a customization.
