Change request is skipping approvals

For only some changes the approvals are skipping. It is happening only sometimes. So it is difficult to re create it non prod environment to check whats wrong with it. I have checked the approvals groups and the member in the groups are active and have approval_user role too. How can i identify the main problem here?

1)Have you checked the workflow context?? is it executing approval activity and skipping or not executing the approval activity

3)Check you have groups memebers in the group

2)Check group members are active and not locked out
