I need to create different change task depending upon response to question asked in change risk assessment.
Can someone please help me understand how this can be achieved?

Probably need more context here. There are several ways to accomplish this. A flow in flow designer probably makes the most sense. It can also be accomplished through a Business Rule or a Script Include, depending on the use case.
