This requirement should work like:
Once you select the yes answer for the first quetion then next quetion should display.
Like that when you select the yes for second quetion then only third quetion should display.

Why use decision builder for this? The solution is already in place. It's called catalog ui policy.
Or, if it's more assessment-like, you can create a survey that also has this conditional showing/hiding as functionality.
