Flow Designer: Create standard change request

I am trying to create a Standard Change for expanding a disk on a virtual server.

I want to have an item in standard change catalog with a flow from flow designer.

I create the item with the necessary variables, but when I get to flow designer and select the action "Create Standard Change request" (after i have retrieved the variables from the item), I can only fill in Short description and description.

How do I get flow designer to choose the correct standard change template?

I know how to select the flow for a requested item in the catalog, but how do I choose the correct standard change?

The simple way is to create a flow from a service catalog request and use the flow to build your standard change.

In this example I did create a requested item in the technical catalog for adding storage to a server.
I have 2 variables: the server and the amount of storage to be added.
The standard change is coming from the ITSM spoke.

The flow looks like this:

Thank you so much for sharing your flow.  I hope you can help answering the following questions so I can get a better understanding of the workflow.

1.  Standard has templates, if an user submit a standard change through service request, how are they going to choice the template.

2.  What is the relationship between Service Request and Standard change. For example, once the change request is created.  How do you know what is the service request number tied this change request? and the what change request number belongs to what service request?

3.  What is the Red icon on action 3 mean?
