How can i add incident number and some description on top the survey widget.

We have a requirement to show the incident number and some description on top of the survey widget.

Our instance was configured to show assessment (Survey) to the user who raised the ticket, once the ticket gets closed. 

For now, we are using a custom page and adding a OOTB Widget (Surveys and Assessment) in to it. But the survey appears to be generic and doesnt show to what exactly we are submitting the survey for. So we wanted some incident and short description to be appeared in the page or widget. Also make some small changes like removing asterisk before a word. 

I am aware that there very limited things we can do from OOTB widget. So i tried cloning it and testes several scripts in HTML and Server. Nothing made any changes to it. 

There was one more widget (Take Survey Deprecated) which shows the incident and short description on the survey page. I would like the Surveys and Assessment to appears as Take Survey Deprecated.

Huh, @Kvb Kiran! I am not that skilled about surveys 😄 but you can try to play around (in dev, eventually pdi) to see how some changes are reflected.

 

Make the changes in the survey designer and then there's preview button:

So if you want to see how it will look with some option true/false, this is the fastest way how to check it.

 

Also, it might depend on what table is is associated with - incident, demand, interaction, etc...

Just for your information, i might have noticed a bug with the widget's functionality. Surveys and Assessment widget do have an inbuilt functionality to show incident number along with short description. After lot of testing, i came to an understanding that If we use introduction/end notes, it conflicts with the widget's functionality, failing to display neither the introduction text nor the widget's functionality.

Checked in a fresh Zurich PDI. Maybe i will try raise a case.

if you think it's a bug, then it shall be reported to ServiceNow from the Support Portal because SN might not be aware of it and if nobody reports that it cannot be ever solved :)) while investigating they might provide a workaround or explain that it's not bug; more possible scenarios..

 

In case you'll raise that case, please share the outcome here for future readers
