I am not able to find Yes/No field type on the incident form layout.
How do I add a Yes/No field

I don't believe you can find the Yes/No type field in the table, It's available only for Service catalog variables
Instead of Yes/No you can try True/False type fields
For example you can refer to the "Active" field on Incident form

Hi,
you can create field of type Choice
Add Yes and No as choice values

Kindly go to form layout and drag a active field(boolean) on that form layout.
