You can build it using Form Builder. Just open Form Builder, pick your table (or make a new one), drag and drop fields, and hit save. Super simple.

Hi @SrinivasS380004 ,

 

Please go to the table management module and create a custom table- your form will be ready already. Now if you want to customize form design (Move filed here, there or adding sections etc) then you can utilize form builder.
