How to call decision table API from script include and return output

Hi Team,

I have created one decision table where I have given input as a "country", "location", and "employee type" and I have set in results "type" as home or office according to all 3 inputs.

Now I have to call the decision table from the script include and return output in the client script. and there I have to auto-populate the value on the form.

can someone please let me know how I can achieve this?
