We have a requirement to configure feedback option on service catalogs similar to options on knowledge articles
Like Feedback capture; helpful yes/no button; ratings.
Please help me if anybody has an idea about this.

May I know what use case it fulfills? It is pure customization and needs to create a similar table to capture feedback. Better create a new catalogue item to get feedback.

So for that use the Survey, after the close of RITM, the survey need to trigger. The survey is OOTB.

Requester wants the feedback option on every catalog form where users can submit their feedback after using that catalog form. whether it is useful or not.

Hey! Would this be possible to implement but on catalog items page, before ordering? So that it captures catalog item name, owner, etc. besides the feedback. Thanks so much
