How to find out if any template used while creating incident? please suggest.

Template means? what do you mean?
Whether incident was created through catalog item? or created through native view? do you want to know about these?

Hello @Mukesh Singh,
You can create a new field in your incident table called "template used?" (checkbox).
Then, in your templates, set that field to true.
With this, you can report on incidents to see in which ones you used a template.
