how to insert data by widget

hello all 
my requirement is that i have one custom table , name is u_practical_demo  in this table there are fields name are 
Shortdescription,Description,FirstName,LastName,Email,Phone ,all fields are string ,  now i want to create record from widget into my custom table how to achive this i am new in service portal any help will be very appreceated

You can use Record producer for this. Please have a look OOTB Record producer for incident.
