How to populate checkboxes on the catalog form on serviceportal

How to get checkboxes checked once they raised form after one year they have to raise again then existing fileds and check boxes check should populate automatically in the catalog form. I used script include and client script it’s not working.ow

No it’s not working, that fields are check boxes ✅.

You need to add gs.addInfoMessage logs to the Script Include to see what is the detected value of these fields from the Glide Record, and an alert to the client script to confirm the value that is being passed back to it.  Also ensure these 3 variable names are exactly correct (case-sensitive).  With the correct logging you will see what is not working.
