How to roll back to same approval

In normal change once the change is moved to scheduled we have ui action called "Reschedule" which will ask to select new Planned and End date and reason once we filled the data and submit it again flow start from new state.
This time approval need to trigger and state has to change once if you click on request approval ui action button but its not triggering the approval but state is changing. As approval is already happen once so to make sure triggering approval once again i want to use roll back activity so that again flow starts as usual from beginning.
how to add this activity in my flow and i am attaching the flow chart here. Please give any idea!!

If you don't mind try the Revert to New in PDI and see the workflow , and then compare and with your WF to find the issue/ gaps.
