Hello @chercm
See flow not attaching on catalog item can be various reason.
1) Flow is not activated.
2) Flow is not mapped to catalog item.
3) Flow trigger condition are not matched.
4) If we change or modify ootb workflow [Service Catalog Request] over Request [sc_request] as this set the approval over Request and on basis of this only OOTB BR [Start FlowDesigner Flow] is triggered which fire off the flow if associated with item.
Now in your case ootb workflow was missing and that is reason your flow was not getting attached to request we submit from portal.
Anybody, if you are reading this post answer check this thread.

Hi @chercm
Everything looks good here. Now 2 things
- Create a new flow and test it, and don't add admin as an approver. Select any other user.
- Log a Now Support case.
If my response proves useful, please indicate its helpfulness by selecting " Accept as Solution" and " Helpful." This action benefits both the community and me.

As Atul mentioned, the approval is on the RITM record. It will not appear on the REQ.
Check on the RITM and see if the approver is listed.

Hello @chercm
Are you sure flow is getting attached when item is submitted?
If no then see if flow mapped with required catalog item.? If done properly see is request getting auto approved as there some oot br which triggers flow workflow if request is approved if not flow never fire off.
If still issue i would recommend to check active context of flow and see what is runtime value pass for approval actions.
If my response has helped you hit helpful button and if your concern is solved do mark my response as correct.
