Impact Assessment Form?

I am currently working on enhancing our Change Management process within ServiceNow and noticed that our existing Risk Assessment Form only evaluates the risk score within a Change Request. I would like to develop an Impact Assessment Form to accurately assess and score the impact of changes. While I have accessed the Assessment Designer, it appears to be configured solely for Risk Assessment. Could you please guide me on how to create an Impact Assessment Form using this tool, or suggest any alternative methods? Or perhaps a way to enhance our current Risk Assessment Form that would address impact of changes, and allow those questions to calculate the impact of those changes?

There is no alternative for this. The maximum you can do is add more questions in the Risk Assessment and assign weight and scores. There is no other plugin that supports fetching risks from outside changes as well."

Do you want to create a fresh risk assessment page? In my previous client, we had the same requirement, but it wasn’t done. Instead, we added more risk questions on the existing risk assessment page. Is that what you’re looking for, or am I still misunderstanding?

Apologies for the delayed response. We have already developed a risk assessment that our change owners are currently using. However, I've noticed that the assessment does not include a score for "impact." I'm wondering if there's a way to incorporate an "impact" score into the assessment.
