Hi
If you created a survey for the incident and provide the condition as mentioned above then you can resolve one of the incident and then go to your 'Ready to take' module under 'Survey Instance'.
There you can check if your survey appears in the list or not. This list shows surveys that are triggered but not yet completed.

Hi Keval,
Did you try the solution proposed by me. There is OOB functionality which can be re used here and you do not need to configure anything custom here.
Let me know if you are stuck some where.
