Incident to major incident proposal

While proposing the incident to major incident. We get one Popup, I want to add some other fields to the popup. How can I do that 

Below is the popup which appears when we click on propose to major incident

It is maintained in System UI -> UI Pages -> mim_propose

Unless it is business critical, I would recommend to keep it out of box and adhere to standard MIM process.
