Need to create Dashboard for Change as per attachment
Please provide the steps in New and Classic UI

These are single score reports which has been added in Dashboard.

You can look ootb change Dashboard.

Go to Reports in navigation 

Click on new report 

Add change request as table

In type select single score.

You need tp create different reports based on conditions.

As @Dr Atul G- LNG  mentioned m this is several single scores added to a dashboard. Where do you face issues in the creation of these.
