New Card & Field on Problem table

Hi, 
I have to create a new card in a problem form.

And here is no any problem with that, but I have to add a new field to this card with name "Root Cause"

and that field should look like this one for example:

If the first line will be completed it should be possible to add another line/record/option below.

I do not have to archive this data to any table just to know.

Look I've got something like this:

But I can't add a new option where the name is more than 1 word, doesn't recognize the "space"?

also I just wanted to get couple new column like created_by  description etc. 

Any Tips ?

Something new for me, could you please tell me what is New Card and how you get this on form?

I mean, Root cause, resolution information, Other information these are a cards.
That's how I identify these objects 🙂

Okay, @DIVI1 So what you are trying to achieve on this? Do you want to add a new field?

Exactly, I want to add a new field to this view/card.

also add some column like description, created_by just how it look like for example on the incident list.
