On Time Cards, when is the time card 'Processed'?

On Time Cards, when is the time card 'Processed'?   We've seen when the time card is approved, it can go into a Processed state or stay in an Approved state.   Unable to see when, why, how it is happening.

Time Cards switch to "Processed" when ServiceNow has automatically generated an expense line for an approved time card (to be used with Cost Management). I *believe* (I'll look closer) that OOB SN should process every approved time card using either the rate defined for the user or a default rate.

There's a business rule on the time card table that controls what gets processed. Only time cards in the Approved state where the task field is not empty will get processed. In the second image you see that the time cards where the task field is blank are still in Approved state.
