One survey is triggring multiple times

When the state is closed the Survey is triggring multiple times.
Can anyone please help on this, It will be usefull.

you said you duplicated the OOTB trigger condition right and in the new one changed the table right?
hence I suggested this approach
share screenshots of both the OOTB trigger condition and the one which you created fresh

I said, Duplicated the notification not the trigger conditions and all the survey trigger conditions are newly created and not dupliocated from oob.
Used OOB notification as below: Just cloned oob notification and updated the HR case table and remins same (When to send, Who will receive and What it will contain).

When i updated the state is closed complete and assignment group is "xyz" in HR case form it is triggering two times to opened for user.

So which OOTB notification you cloned?
Sorry but I am not getting what's the exact requirement is
1) trigger conditions are OOTB, you didn't do anything to it
2) share OOTB notification & new notification screenshots

My requirement is simple, When the state changes to close complete and assignment group changes to XYZ group the survey should be trigger. so below are the steps which i created.
Created new survey trigger condition as below:
I have OOB notification as below:(Inactivated the OOB)
But i duplicated this same oob notification and i renamed and changed the table as below:
But here when i triggering the survey with those conditons are met the survey triggring twice.
Can you please help to avoid duplication.

so did you check both the emails got triggered from same notification as the time is same for both
