Pop up to show a set of checkboxes when user clicks on ui action

On any "normal" change in state earlier than "scheduled"

ui-button appears for group "Change Enablement Assurance"
when clicking the button a popup appears with a set of checkboxes.
each checkbox will add to a template email produced.
one final checkbox for "REJECT" that will reject the ticket (if selected - on submission the change is rejected and reset to new)

once the template is ready - "Change Enablement Assurance" should be able to edit the email before it is sent and submit for the email to be sent to the "Change Coordinator" & "Requestor" & work note added to incident

** Check if there is any way to indicate on the change record that a certain field was marked as non-compliant **
