Reason description field in walk-up appointment widget is not visible

Hello all, how can we show the reason description field in the walk-up appointment booking widget?

*I need a reason description field under what is your reason for visiting?

@Kishor O  -While configuring the Reasons for Visit, select the Text Entry Check box, it will add the Description field.

table name: wu_m2m_location_queue_reason

You need to make text entry = true for Reason for Visit related list in walk-up location
