REQ and RITM is getting closed but the active checkbox still showing as ture

Hi,

 

i am facing issue while closing the TASK > RITM > REQ. issue is when i will close the task then as per the flow RITM will wait for user response.  before putting any response the RITM and REQ is getting closed.

can any one help to get the answer is query.

and also active checkbox is still showing as true. can you please help to get the reason behind this. 

in my case this happening for any specific catalog item. Name of the catalog item is asset update and approved software request. for these catalog item it is not working.

@Inds Please check the flow/workflow associated with the catalog item in question and see if it is responsible for the issue.
