Risk Assessment related link not displaying

Wondering if anyone can help me out. I've been looking into Risk Assessments and how they work (method for using a questionnaire to then generate a risk score)
I've setup a Risk assessment as per documentation, but I can't seem to understand how and when it displays for use. Obviously confusing since as part of the setup you define the conditions. Experience so far as I cannot see the related link anywhere, and then randomly it just appears on a Change I don't expect it to.
Below is how I have the conditions set on the assessment
Above this condition set, it's telling me only one record is found matching this condition. But even going to that record, the related link does not appear.
I'm not going crazy here right? That condition set should trigger against any Change where Category is Service, and State is one of New,Assess or authorise, and Active is True.

Hi Jack,
Thanks for the response. As per my OP, I followed the documentation to activate this, and I've seen it work in some isolated instances, can't follow the logic or reason as to how it's just stopped working.

Hi Daniel,
Risk Assessment is a related link, so you need to check UI Actions available on Change Request Table.
Please check condition to know when this Link is Available on form. This UI Action is used as form link:

Checked this when I first started having the problem and can confirm the UI action is in place and active, was done as part of the plugin activation.
