Risk assessment showing blank window

I have created a new risk assessment and added the questions using the Assessment designer, however when i click the "Risk Assessment" ui action in the change request, it shows blank screen. Please guide what i am missing here.

It works with the exsisting risk assessment. Only issue with the newly one i have created
