Risk assessment tab missing

I have change in assess state but the risk assessment tab is missing. I need the risk assessment tab.

Please help me with your inputs !!

For everyone here not able to see the related list option 'Task Assessment > Task'. I used 'Assessment Instance > Task' and the related list showed up with the completed assessment once I completed it. Hope this helps for future reference!
